Are you passionate about helping people improve their health and wellbeing?

We are seeking a Social Prescribing Link Worker to empower individuals to take control of their health and wellbeing.

About the Role

  • Work with individuals to explore โ€œwhat matters to meโ€.

  • Connect people to community groups and statutory services for practical and emotional support.

  • Support and strengthen community groups to be sustainable and accessible.

  • Work collaboratively with local partners and healthcare providers.

About You

Weโ€™re looking for someone who is:

  • A strong communicator with excellent interpersonal skills.

  • Empathetic, supportive, and passionate about making a difference.

  • Experienced in coaching or health and social care (desirable but not essential).

  • Able to work independently, manage a caseload, and travel across Central Cornwall.

  • Confident collaborating with voluntary sector partners and healthcare providers.

We are particularly interested in candidates with experience in:

  • Type 2 diabetes education

  • Supporting weight loss and lifestyle changes

  • Empowering independence and breaking barriers

  • Supporting employability and life skills

  • Signposting, referring, and goal setting
