The Role
You will deliver high-quality nursing care within a busy primary care setting, supporting patients with long-term conditions, preventative healthcare, health promotion, and routine nursing procedures. Working both independently and as part of a multidisciplinary team, you will help provide safe, effective, patient-centred care across our practice sites.
Key responsibilities include:
Chronic disease monitoring and management
Venepuncture, ECGs, wound care and dressings
Immunisations and vaccinations
Health promotion and lifestyle advice
Telephone patient support and triage
Supporting and mentoring healthcare assistants and students
Maintaining accurate patient records using SystmOne
What We're Looking For
Essential:
Registered Nurse (NMC)
Strong communication and IT skills
Experience with clinical procedures such as venepuncture, ECGs and wound care
Ability to work independently and as part of a team
Desirable:
Experience in Primary Care
Knowledge of diabetes, asthma, hypertension or other chronic disease management
Immunisation and cervical cytology qualifications
Independent prescribing qualification
