Are you a registered Pharmacy Technician looking to develop your clinical role in Primary Care?
St Austell Healthcare is recruiting a Pharmacy Technician to join our expanding multidisciplinary team and support the delivery of our new Hypertension & Lipid Management Service across our surgery and Primary Care Network.
Working alongside our experienced Clinical Pharmacists, you will help patients manage high blood pressure and cholesterol, promote medicines optimisation, support cardiovascular disease prevention, and contribute to improving long-term health outcomes across our local population.
Key Responsibilities
- Conduct telephone and face-to-face patient consultations.
- Support patients with hypertension and cholesterol management.
- Review home blood pressure readings and monitoring results.
- Promote medicines adherence and healthy lifestyle changes.
- Coordinate blood tests and follow-up monitoring.
- Undertake medicines reconciliation and medicines optimisation activities.
- Maintain accurate records within SystmOne.
- Support achievement of QOF and prescribing quality initiatives.
- Refer patients to Clinical Pharmacists for prescribing decisions and complex medication reviews.
About You
You will:
- Be registered with the GPhC as a Pharmacy Technician.
- Have experience working in a pharmacy or healthcare setting.
- Possess excellent communication and organisational skills.
- Enjoy working within a multidisciplinary team.
- Have an interest in cardiovascular disease prevention and medicines optimisation.
Previous Primary Care or SystmOne experience is desirable but not essential.
