Clinical Pharmacist (Band 6)
Job Summary
We are seeking a newly or recently qualified pharmacist to join our Primary Care Network (PCN). This is an excellent opportunity to develop clinical and professional skills in primary care, working alongside experienced pharmacists and technicians. The role focuses on medicines optimisation, long-term condition management, and responding to medicines-related queries, while supporting safe prescribing, QOF targets, and PMOS.
Main Duties
- Deliver safe, effective, patient-centred pharmaceutical care under senior supervision
- Support medicines optimisation, medication reviews, and prescribing
- Manage medication queries from patients and healthcare professionals
- Conduct medication reconciliation and review repeat prescriptions
- Support long-term condition management and high-risk medication monitoring
- Contribute to audits, quality improvement, and medication safety initiatives

Role Details
- Full-time (37.5 hours), part-time considered
- Salary: Band 6 (dependent on experience)
Key Requirements
- MPharm (or equivalent) and GPhC registration
- Understanding of medicines optimisation and prescribing guidance
- Strong communication and teamwork skills
Desirable
- Primary care experience
- Independent Prescribing qualification (or willingness to train)
- Experience with medication reviews, QOF, and multidisciplinary working
