Chronic Disease Nurse

St Austell Healthcare is seeking an experienced and motivated Registered Nurse to join their proactive team as a Chronic Disease Practice Nurse, specialising in diabetes management. The post involves delivering evidence-based nursing care to a patient population of approximately 37,000 across multiple sites. Responsibilities include managing long-term conditions, performing clinical tasks (such as venepuncture, immunisations, ECGs, spirometry), providing health promotion and education, mentoring healthcare assistants and students, working autonomously, and contributing to practice protocols and clinical audits. The ideal candidate will be proactive, take clinical responsibility, and demonstrate strong decision-making skills. A Nurse Prescribing qualification is desirable.
