COPD Support Group Membership

Healthwise membership gives you access to swimming, gym, classes and the COPD support group. You also have access to a healthwise trainer to help you with a personalised exercise and support plan.

A new COPD Support Group weekly session on Tuesday 27th January 10.30am.

Session Information:

The session will be delivered by Rebecca - Healthwise Facilitator

The session is aimed at patients with MRC Grade I & 2

Referral will use the existing Healthwise Referral process, those patients being referred into Healthwise that have COPD MRC Grades 1&2, will then have the additional benefit of being able to access the support group.

Format of the session:

10mins Welcome (How week been)

40min activity (could include chair-based; low class; walk)

10 mins chat & cuppa at the end.

St Austell Leisure Centre Walking group (Ramblers)

Polkyth Walk

As you arrive please make yourself known to any of the walk leaders who will be wearing a hi vis vest. The walk is a friendly, community based group organised by the Social Prescribing team at St Austell Healthcare. Along with the benefits of walking you can enjoy spending time outdoors talking with others and meeting new people. The walk itself can range from 25-40 mins at a fairly slow pace.

The walk is every Monday at 10:30am so please arrive just before. Take into account the need to park if you're driving in. All walkers meet inside the Polkyth Leisure Centre at the cafe.

Please note the walk may be cancelled if weather warnings are in place on the day.

Pinetum Gardens Walking Group (Ramblers)

Pinetum Gardens Walk

As walkers arrive they split into 4 groups with different walk paces to cover the 30 acre site at a speed to suit each individual. Please make yourself known to any of the walk leaders who will be wearing a hi vis vest. Along with the benefits of walking you can enjoy spending time outdoors talking with others and meeting new people.

Slower Pace Walk - 30-40 mins

Faster Pace Walk - 45 mins

The walk is every Wednesday at 10:30am so please arrive just before to give you time to walk to Pinetum Cafe where they meet.

Mental Health Support

Talking Therapies (IAPT/OSW)

The Improving Access to Psychological Therapies (IAPT) service provides free, confidential talking therapies for people who are registered with a GP in Cornwall and the Isles of Scilly, aged 16 years and over, experiencing common difficulties including: low mood, stress, anxiety, worry.

We can also support people with long-term conditions, which affect their mood, stress and anxiety. This includes:

chronic obstructive pulmonary disease (COPD).

We help patients better understand the links between physical health and mental health. Also, how the specifics of your condition, alongside its treatment, are intertwined with your mental wellbeing and vice versa.

Referrals - You must be registered with a GP surgery in Cornwall or the Isles of Scilly to use the service. You can self-refer online or call 01208 871 905. Appointments are by phone, video or face-to-face. Please note that if your self-referral raises concerns about your safety, we will signpost you to a more appropriate service. You can also speak to your social prescriber, if you have one, and explore whether this service may be a suitable option for you.

Community Energy Plus

As the seasons are changing and we look towards the colder months ahead, we wanted to reach out to our referring partners to provide a reminder of the services we provide, and how to work with us.

Community Energy Plus (CEP) is Cornwall’s dedicated energy advice charity, offering practical help and guidance to householders across all housing tenures who may be struggling with energy costs.

We offer free Energy Awareness Training for frontline workers who support vulnerable householders across the county, as well as free Presentations to Community Groups. Our frontline worker sessions are designed to help your team feel more confident when speaking to clients about energy issues. We can deliver the training in-person or online, depending on what works best for your team.

Our Energy Advice Service provides:

Help understanding energy bills and managing energy debt

Top-up vouchers for households in financial crisis with pre-pay meters. Referral must be received from a frontline organisation supporting the client. There are a limited number of vouchers available

Emergency support to keep warm (heated blankets and emergency top-up vouchers for householders with pre-pay energy meters who are referred to us by a frontline organisation)

Help to pay energy bills – we deliver the Household Support Fund on behalf of Cornwall Council. Householders living in any tenure who are struggling to afford to keep warm can apply. Please note that Cornwall Council will make automatic Household Support Fund payments to targeted households in early November. To prevent duplication, we do not provide energy payments for households with multiple children who receive Council Tax support as these households will automatically receive £140 per child.

Access to grants for heating repairs, insulation, and energy efficiency upgrades

Advice on reducing damp and mould

Referrals for free home fire safety checks and alarms

Signposting to income maximisation support and discounted water tariffs

Links to trusted local and national organisations for further help
