The aim of the project is to support residents by placing Community Health & Wellbeing Workers in specific areas of St Austell. If you are not on one of our roads then please refer yourself to the Social Prescribing Team who will be happy to help. They can be contacted on: (01726) 626844 or email: social.prescribing1@nhs.net

Our Community Health and Wellbeing Workers can explore with you and your household:

  • Increasing your social network.
  • Activities to support your health and wellbeing.
  • Accessing mental health support.
  • Helping you receive practical support.
  • Signposting to other services.
  • And much more

We understand that life can be challenging at times. With support from a Community Health and Wellbeing Worker you will increase your support network, gain a listening ear and explore possible solutions to issues you may be facing.

If your household comes under the selected postcodes for this project then you will be contacted by our team, either by letter, postcard or friendly knock on the door.

Please note we will also be talking to residents about opportunities for cancer screening, immunisations and health checks.
