There are many local help services available if you are looking for ways to manage your health and wellbeing. Find out more about social prescribing and the support available in your community.

What is social prescribing?

Social prescribing is a way of linking you to non-clinical support in the community. Improving your lifestyle and making changes can be very challenging. You may not be sure where to start. Social prescribing can help to give you the confidence and support you might need to make a change.

There are a fantastic range of activities and organisations in the local community. Social prescribing is about linking you to these opportunities, with the aim of improving your health and wellbeing.

Getting a social prescribing link worker

There are local agencies, such as local charities, social care and health services that can refer you to a social prescribing link worker. They can give you support and connect you with what’s going on in the community to help you take control of your mental health and wellbeing.

If you would like more information on social prescribing in England, then we recommend visiting the NHS website. The website has lots of resources and further information on the NHS England social prescribing programme.

Community hub

There are lots of local groups, services and activities that you can get involved in. Find out what’s going on in your community.

Help at Hand App

The Help at Hand App provides you with easy access to the latest support services available in St Austell, by telephone and online.
This App is available via the App Store (iPhone, iPad) or Google Play (Android).

The App and most of the services listed on it are completely free of charge.

Our social prescribing team would like to invite you to a group weight management presentation

The session will cover two areas.

  1. NHS advice on how someone can lose weight through diet and lifestyle.
  2. What support is available locally including, weight loss programmes, local exercise opportunities and chances to weigh in regularly and get advice.

The session will not cover medical concerns or medical weight loss interventions e.g. weight loss injections or bariatric surgery. If you wish to only discuss medical details and not diet and lifestyle then please go through our website and request to speak to your GP.
