Digital Neighbourhoods at St Austell Healthcare

Using technology to help us, help you.

Nobody likes ending up in hospital unexpectedly. That’s why we’re using a new NHS digital tool called Brave AI — part of the South West Digital Neighbourhoods Programme — to help us spot when someone might be at risk of becoming unwell. This means we can reach out sooner, offer personalised care, and help you stay well at home, supported and doing what matters to you.

What is Brave AI?

Brave AI is an NHS-approved digital risk assessment tool developed by Bering Ltd, a UK medical AI company. It uses secure, anonymised patient data already held by your GP practice to identify people who might be more likely to need unplanned hospital care in the next 12 months.

If you’re identified as being at higher risk, one of our team may contact you to invite you to a personalised health and wellbeing assessment. Together, we’ll discuss what support might help you stay well and independent.

If I’m identified as ‘at risk’

You may be invited to talk with a member of our Integrated Neighbourhood Team — professionals from local health, social care and community services.

These conversations focus on what matters to you, not just “what’s the matter with you”.

Your data and confidentiality

  • No new data is collected – Brave AI uses existing GP records.
  • Information analysed by the tool is anonymised.
  • Only authorised healthcare professionals can access identifiable information.
  • Brave AI meets NHS Digital Technology Assessment Criteria (DTAC) for safety and security.
Opting out: If you prefer not to take part, please contact our practice team to opt out of the Brave AI review.
The NHS National Data Opt-Out does not apply to risk assessments, so please ask us directly.

What is Artificial Intelligence (AI)?

Artificial Intelligence (AI) refers to computer systems that can identify patterns and make predictions from data. In this programme, AI helps NHS staff review patient information faster and more effectively, so care can be offered proactively rather than reactively.

About the Digital Neighbourhoods Programme

The South West Digital Neighbourhoods Programme aims to use digital technology to:

  • Move care from hospital to community
  • Shift focus from sickness to prevention
  • Help people live healthier, independent lives

Over 190 GP practices across the South West are now using Brave AI to improve patient care and support early intervention.
