If you need to change or update your contact details, you can do so by completing the form below. We'll let you know when your details have been updated.
Email address / phone number
To update your email address or telephone number, please complete the online form here. Make sure to provide your full name and date of birth.
Name / address
It is important that we have the correct contact details for you. If you change your name, please complete the form below. Bring the form to reception, along with the legal documentation confirming your change of name (i.e. Marriage Certificate, Deed Poll). The receptionist will photocopy this original documentation and hand it back to you immediately.
If you have changed address, please send an online request here .
Height, weight and smoking status
It's helpful to have your height, weight and smoking status on your medical record. This information is needed if you have a referral.
You can update these details by completing the form below. You can either hand it in at reception, or email it to reception.sahc@nhs.net
